  • Provide information about health insurance coverage and provide one on one assistance should you need help accessing Vermont Health Connect

Services We Provide:

  • Lab work can be done in our office for your convenience

  • Prescription refills

  • Vaccinations provided by the health department, including flu shots

  • X-Ray is generally available with official readings done by UVMMC Radiologists

  • Coordination of referrals to other health care providers, including mental health needs, or for diagnostic testing. If you prefer to schedule your own appointments, we can support this as well- please update us as to when these appointments occur so we can track and review all results and reports.

Physicals and Preventative Care:

A Health Maintenance Exam, (also called a physical or annual exam) is an important opportunity to screen for and prevent illness. It is an extended visit with your primary clinician that includes an interview, a full physical examination, and counseling regarding diet and exercise, and other lifestyle choices that affect your health. It may include laboratory testing to screen for disease, administration of appropriate immunizations, and referrals for recommended tests. If we find other medical conditions during that visit, a follow up visit may be required to more thoroughly evaluate and plan treatment for those conditions.

Pediatrics:

We care for children with a family-centered approach. Your child's health is best maintained through regular check-ups. We care for well children and children with chronic diseases or acute illnesses. During a Well-Child visit, we evaluate growth and development and screen for illness. We also discuss nutrition, healthy habits and accident prevention. We may discuss the child's social setting and behavior. We make recommendations for appropriate immunizations and screening tests. Well-child visits are usually scheduled at the age of three days, two weeks and at 2, 4, 6, 9, 12, 15, 18, and 24 months. Beyond the age of two, we schedule yearly physicals. School, Sports and Camp Physicals can be part of a Well Child Visit.

If you are looking for a doctor or primary care nurse practitioner for your newborn, please be in touch with us prior to delivery. After delivery, while still in the hospital, the hospital will notify your baby's clinician of the birth of your child and send us records of the delivery and testing done.

Women’s Health and Gynecologic Care:

The FPA doctors and nurse practitioners are trained in general women's health & gynecologic care including Pap testing, Sexually Transmitted Infection screening, contraceptive management and breast exams.

Mental Health Care:

Mental Health is an important part of overall well-being and can have a strong effect on one's physical health. We screen for and treat common conditions, such as depression and anxiety, as well as substance abuse.

Urgent Care:

We offer our patients same-day appointments to diagnose and treat acute illness, such as infections or worsening of chronic diseases. We also suture small lacerations, treat minor burns, and evaluate and treat injuries such as sprains and minor fractures. A doctor or nurse practitioner is available by telephone after hours for urgent medical issues. If you are unsure whether you need to be seen immediately, please call and speak with our triage nurse.

Chronic Disease Care:

As Family Physicians, we are trained to diagnose and treat a broad spectrum of chronic conditions, including, diabetes, obesity, hypertension, high cholesterol, heart disease, asthma, depression, migraines and emphysema. We work with patients to optimize their health through appropriate testing, treatment and referral. We help to coordinate care with specialists if needed and provide continuity and support for them and their families. Routine chronic care visits with the primary physician are scheduled at intervals that are determined by individual patient need and disease guidelines.
